On Tuesday June 3rd Monocacy Health Partners Primary Care hosted ‘KNOW Heart Disease’ the second of four workshops in 2014 focused on population health management. Each of these workshops is FREE and open to any MHP Primary Care patient. As patients entered the workshop they were able to stop at several education tables including one called ‘Eat This, Not That’ hosted by Emily Spear RD LDN, Outpatient Dietitian FMH Wellness Center. The ‘Eat This, Not That’ table contained numerous food models and portion sizes related to a heart healthy diet. Patients were able to ask specific nutrition questions and could view and take a number of recipes provided by the American Heart Association website.
MHP Primary Care’s Medical Director Dr. Koilpillai opened the workshop with a discussion focused on the Patient Centered Medical Home (PCMH) model which is an innovative, team-based approach to providing healthcare services, download PowerPoint Presentation. Our goal is to provide patients with better healthcare by emphasizing whole person care. Patients will find they have easier access and communication with providers through a team of professionals available to coordinate their care.
