Monocacy Health Partners Primary Care hosted a very successful KNOW Diabetes education session on March 19th attended by around 85 of MHP Primary Care’s current patients. Information presented included early indicators of diabetes, types of available care and treatment, and the importance of diet and exercise in maintaining health. Presenters were Poonam Sood, MD and Jacqueline Messner from Endocrine and Thyroid Specialists, Tina Miller and Jessica Mathews from Preventive Cardiology/ProMotion Fitness, and Judy Goffi from Wellness. Lisa Lipton, MHP Primary Care’s Practice Administrator, presented an overview of the patient centered medical home role in helping to maintain patient health and pursue the Triple Aim of reducing costs, improving care coordination and achieving high levels of patient satisfaction. She informed attendees that Monocacy Health Partners Primary Care has been awarded NCQA Level III Patient Centered Medical Home Accreditation (the highest level) at all four practice sites and announced that walk-in hours were introduced at MHP Primary Care’s Rose Hill location in January 2015.
Vendors provided information and portion control plates made available at no charge. Back tables had additional information and real food displayed in appropriate portions for healthy daily caloric and carbohydrate intake.
