On Tuesday March 18th Monocacy Health Partners Primary Care kicked off “KNOW Diabetes” the first of four workshops in 2014 tailored to a high risk patient population. Each of these four workshop is free, open to any MHP Primary Care patient and focused on population health management. MHP Primary Care’s Medical Director Dr. Koilpillai, opened the evening with a discussion focused on Patient Centered Medical Home (PCMH). The Medical Home is a concept – not a place. It is an innovative, team-based approach to providing healthcare services. Our goal is to provide you with better healthcare by emphasizing whole person care and coordination of care. Linda Mino CDE, Emily Spear RD and ProMotion Fitness+ engaged patients in interactive discussions about diabetes prevention and management, nutrition and exercise. Our “Eat This, Not That” nutrition table was a big attraction for patients. Approximately 490 type 2 uncontrolled diabetic patients were invited to attend the first workshop. Every patient evaluation rated the overall program as 5/5 or Excellent. Stay tuned for more workshops.
